District Nurses/Health Visitors/Social Workers
The above professionals may be approached directly without seeing the doctor first and will be able to give expert advice on their own subject.

The health visitor is a qualified nurse and has been specially trained on all matters relating to young children’s health. She can be contacted through the GP practice.

The district nursing team provide skilled nursing when needed, in the patients own home.

The midwife will be available throughout your pregnancy and can provide all aspects of care.

Referral for further care
Arrangements can be made through any of the doctors for referral to other primary care team members:

• Dietician
• Physiotherapy
