For the safest and fastest prescription processing, we recommend using EMIS Patient Access. If you cannot register for Patient Access, you can complete our standalone Repeat Prescription Request form online.

We highly recommend utilising EMIS Patient Access over simple form requests because it integrates directly with your secure personal medical records, lists exact due dates, and provides historical verification to minimise administrative delays. Register for online services.

If you have been unwell for 7 calendar days or less, your employer must accept an Employee Self-Certification (SC2) form, which does not require a doctor's signature. For absences longer than 7 days, complete our onlineDoctor's Sick/Fit Note (More than 7 Days) Request form.

To keep patient details current, please utilise the Change of Personal Details online form. If your address is updated, our admin system automatically checks whether you still fall within our approved Kensington Medical Centre practice boundary.

To access online bookings, messaging, and repeat ordering, complete the online Register for Online Services form. Our reception desk will verify your identity before emailing secure system credentials.

Simply fill out our Nominate a Preferred Pharmacy online form, naming your chosen Belfast chemist. All subsequent prescription approvals will automatically transfer directly to that chemist.

Use our onlineCancel an Appointment form or send a cancellation command via EMIS Patient Access. Letting us know at least 24 hours beforehand lets us hand the open time block to someone else who needs urgent consultation.

Submit the online Register a Carer form to register. Flagging your carer status enables our systems to prioritise you for yearly influenza vaccinations, and direct you to tailored medical carer networks within Northern Ireland.

Repeat prescription request forms are finalised within 48 working hours (2 working days). General updates and non-clinical administration takes 3 to 5 business days, while intricate medical and insurance report requests can take up to 21 working days.

Yes! If you reside within our practice catchment area in Belfast, you can submit the official New Patient Registration Form through our website along with digital images of the required identifying documents.

Yes, you can submit requests for insurance or employer examinations using ourMedical Report Request form. Please note that certain specialised assessments are classed as non-NHS private services, and standard fees will apply.
